2017 Award Nomination Form

Individuals, athletes, coaches and MSOs/PSOs are invited to submit nominations for the following awards:

1. The Bob Secord Award: this award recognizes an athlete, coach or official who best exemplified the spirit of the ParaSport Winter Games in Brantford by being a role model to participants and demonstrated camaraderie and leadership on and off the field of competition.
2. Coach of the Year Award
3. Female Athlete of the Year
4. Male Athlete of the Year
5. The Robert W. Jackson Award: this award recognizes an outstanding leader who has exemplified extraordinary leadership, dedication and excellence in sport for athletes with disabilities.
6. The Ron Foster Award (aka Volunteer of the Year Award): this award is designed to recognize a volunteer at the provincial level who has dedicated their time and energy towards the delivery of opportunities for athletes with a physical disability in sport and recreation.
7. Rookie of the Year Award: this award will be given to an up-and-coming high school para-athlete.
8. Team of the Year Award
9. The Unconquered Athlete Award: this inaugural award will be given to an athlete who has overcome a life-altering physical or mental disability by participating in sport.

Instructions:

• Please submit your completed application to [email protected] by no later than Friday, April 21, 2017. Your e-mail must include:
  • the name, valid e-mail and phone number of the nominee;
  • your name and contact information;
  • the award for consideration; and
  • no more than a one-page write-up describing why the nominee is deserving of the specified award.
• Athlete, team and coach nominees must be in good standing with their respective MSO/PSO and with ParaSport Ontario.
• Selections will be made by the Evening For Inclusion Awards Selection Committee based on the nomination forms and write-ups provided.
• Please send any questions to [email protected] All award recipients will be notified by no later than Monday, May 1, 2017. The recipients will be invited to attend the Evening For Inclusion Concert on Thursday, June 1, 2017 to accept their award.
