Survey: Health Quality Ontario is looking to better understand the experiences of patients who are leaving the hospital to recover at home (home also includes supportive housing, shelters, long-term care facilities and nursing homes).
We want to hear from patients—and their families—who have been admitted to an Ontario hospital overnight sometime in the last three years.
