Join us from 10:00 – 12:30 on Monday July 30, 2018 for a housing training held at the Community Room in the Montgomery County Human Services Building, 1430 DeKalb Street, Norristown, PA.
This training is recommended for NHT coordinators, autism service coordinators, mental health caseworkers, children/youth caseworkers that work with kids with disabilities aging out of foster care, ID housing caseworkers, and hospital liaisons.
Attendees will receive an overview of Montgomery County housing options, and also an introduction to 811 project rental assistance, which provides a permanent supportive housing subsidy for eligible individuals. The training will cover 811 eligibility, priority populations, application process, and reporting requirements for service providers.
811 Priority Populations
Institutionalized, but able to live in the community in permanent supportive housing.
At-risk of institutionalization with no permanent supportive Housing
Congregate Care Setting and desires to live in community

Individual must be 18 – 61 at the time of lease up
Individual must have disability
Individual must be MA eligible in the community
Individual must be at or below 30% Area Median Income
